Introductory Statement for Resuming Work & Sign-off
The signature confirms that the employee has confirmed that he/she has been successfully reintegrated into the working world after the COVID-19 leave and that he/she is able to return to his/her job with full commitment and in accordance with the company's drug and alcohol policy. If this or an alternative proposal is not confirmed, the reason for this or an alternative proposal must be listed below and sent to Human Resources for approval.
