Site Inspection Checklist

1. PLANT AND EQUIPMENT

1.1 Rotating amber beacons fitted and operational

1.2 Reversing alarms fitted and operational on all mobile plant

1.3 ROPS / FOPS fitted to plant

1.4 Daily operator pre-start checklists being completed

1.5 Seat belts being worn by operators where required

1.6 Operating procedures as per SH&EWMS being followed

1.7 All operators certified and trainees on log books and supervised as necessary

1.8 Permit to operate sticker current

1.9 Operator inducted to site (photo ID)

2. ELECTRICAL EQUIPMENT

2.1 Leads and power tools within test period

2.2 All leads are off the ground on lead stands or placed as not to sustain damage

2.3 Are residual current devices being used

2.4 Power tools and leads are in good condition and no damage taped up

2.5 Power leads do not exceed 25m for a 1mm core size

2.6 All power leads are "commercial" type

2.7 All power supplies protected by RCD, tested daily on generators

3. TRENCHING AND EXCAVATION

3.1 Personnel " tool boxed" on SH&EWMS for tasks / activities

3.2 Trenches / Excavations have been benched / battered / shored

3.3 Supervisor/competent person inspecting trench/excavation before personnel entering

3.4 Safe access/egress i.e. access ramps every 9m - ladders every 9m firmly fixed at the top

3.5 Spoil kept minimum 600mm from edge of trench / excavation

3.6 Benching not greater than 1.5m in height (unless certified by engineer)

3.7 Adequate signage / barricading to prevent falls / entry to excavation tenches

3.8 Pier holes adequately protected or barricaded

3.9 Machinery operating minimum of 1m from edge of trench / excavation

4. LADDERS

4.1 Ladders are marked industrial use only on AWX property timber / fibre glass reinforced

4.2 Ladders extend 1m above access / working platform and securely tied off

4.3 Marked industrial and a minimum SWL of 120kg

4.4 Placed on firm level ground

4.5 Persons not working off ladders

5. SCAFFOLDING

5.1 Over 4m erected by licensed scaffolder. Scaff-tag system used (if it is a site requirement)

5.2 Guardrails 900mm - 1100mm above the working surfaceMid rails 450mm - 600mm above the working surfaceToe boards 150mm minimum height

5.3 Access ladder or stairs free of debris, dirt etc

5.4 Decks fully planked and secured

5.5 Sole plaes founded on sound base

5.6 Adequately tied off / maintained

6. SAFETY BELTS AND HARNESSES

6.1 Where adequate working platform cannot be supplied, are static lines, harnesses, lanyards, reels available

6.2 Fixing points for static lines have been certified to meet 15k/h

6.3 Persons required to work while wearing industrial fall protection have been trained and can produce evidence

6.4 Persons required to work at height using industrial fall protection have been "tool boxed" on the Safe Work Method Statement for the tasks/activities

7. PERSONAL PROTECTIVE EQUIPMENT (PPE)

7.1 Safety helmets

7.2 safety glasses and long sleeve shirts being worn

7.3 Protective footwear

7.4 Hearing protection

7.5 Respiratory protective eqipment

7.6 High visibility clothing, reflective for night works

8. OXY ACETYLENE WELDING / CUTTING EQUIPMENT

8.1 Regulator, hoses, handset condition

8.2 Flashback arresters fitted to the gauges end for civil construction and both ends for mining

8.3 Stored away from electrical contact

8.4 Bottles stored in ventilated/secure bottle store

8.5 Bottles stored upright

8.6 Not stored in stairways, egress routes or confined spaces

8.7 Appropriate hazchem signage

8.8 Fire extinguishers available

8.9 Bottles securely fixed to trolley

9. CHEMICALS

9.1 Material Safety Data Sheets (MSDS) available at the workplace

9.2 correct labelling and packaging / storage

9.3 Suitable waste disposal

9.4 PPE provided

9.5 Eye wash available (First Aid Kit)

9.6 Hazard signage displayed

9.7 Fire extinguishers available

10. ADMINISTRATIVE

10.1 First aid boxes in all AWX vehicles / crib sheds, maintained regulrly

10.2 Amenities cleaned and stocked daily Adequate amenities for number of workers

10.3 Crib rooms cleaned daily Adequate room for number of workers

10.4 Toolbox talks carried out weekly / fortnightly

10.5 Register of injuries in Injury / Accident book at each First Aid Station

11. HOUSEKEEPING
