Scaffolding Safety Checklist

Use this template to inspect scaffolding post-installation to ensure it is safe for use. Begin the checklist by capturing general information about the structure. Ensure the foundation and base of the scaffolding are secure and check that the height is within safe dimensions. Inspect the platforms, bracing, tubes, and standards for proper installation and capture any defects. Check the load capacity and access points for the scaffolding. Does each stand have a base plate?
Are sole plates of the correct dimension being used?
Height
Is the free-standing tower scaffold steadied with guy wire according to regulatory requirements for its height?
Are safety cages needed around vertical ladders based on height according to regulatory requirements?
Is a perimeter placed on work surfaces (toe board permanent and temporary height) according to regulatory requirements?
Is perimeter handrail heights with a mid-rail around all work platforms according to regulatory requirements?
Are rolling scaffold wheel brakes locked and outriggers extended to maintain maximum height of 3 times the smallest base dimension?
Are toprail based on height according to regulatory requirements?
Are midrails installed approximately halfway between the toprail and the platform surface
Geometry
Are the standards vertical?
Are the ledgers and transoms horizontal?
Are the joints in the standards staggered?
Are the joints in the ledgers staggered?
Are the transoms correctly spaced?
Platforms
Are the supports correctly and evenly distributed?
Does the overhang have the minimum standard size?
Does the board have a creaky sound?
Are lapped boards safe?
Are boards secure?
Bracing
Is the ledger bracing fitted on alternate pairs of standards for the full height of the scaffold?
Is the façade bracing fixed to every lift?
Is the façade bracing fixed to the face of the scaffold at standard intervals?
Is the façade bracing set at standard angle
Tubes and Fittings
Are the tubes and fittings used in good condition?
Have the correct couplers been used?
Are all fittings tightened sufficiently?
Are check couplers fitted as required?
Access
Is access provided to the full height of the scaffold?
Are the ladders positioned at standard angle?
Is the head tied and the feet on a firm base?
Do ladders rise in standard height above platforms?
Loading
Is the scaffold being used as intended?
Are loads on the scaffold evenly distributed?
Are stacked materials secure?
Ties
Are sufficient ties used in accordance with safety regulations?
Are ties fixed with a double coupler?
Are ties fixed on both inner and outer ledgers or standards?
Guard Rails and Toe Boards
Are the rails fixed at least 950mm above the platform?
Is the rail fixed a maximum of 470mm above the toe board?
Are toe boards fitted to all platforms and at least 150mm and secure?
Scaffolds Tag
Is the scaffolding tag displayed at the access point?
Has the front of the scaffolding tag been completed correctly?
Have the inspections been written in and signed by the authorized person?
Are the inspection frequencies in accordance with the standard regulations?
