8ft of 10ft ground rod buried at least 2.5ft into ground
PVC ground rod covers in place for visibility
Spotters have on Hi Vis vest when spotting equipment
Sufficient quantity of absorbent materials on location
75ft buffer zone from all pressure sources
MSDS for all chemicals on location
Fire extinguishers out and hanging on front of equipment
Trip hazards clearly marked
Appropriate signage installed
Shower trailer set up at minimum 55ft from chemical area and function tested
All containers labeled
Equipment physical appearance (rust, paint, etc.)
Iron certifications within annual requirement and banded
All JSEA's signed
Fuel trailer grounded and bonding cable installed and available for use when fueling
Employees have correct PPE available for use
Extra PPE inventory available on location
Are wheel chicks available for vehicles
All containments in place / sides upright/ bridges installed
Lifting equipment is inspected and register up to date
Fall Protection inspected and register up to date
Controlled work instructions on site

BOP
Current BOP Certification
BOP has pressure test within 14 days of arrival
BOP function test
Shear ram performance test within previous 180 days
Confirm BOP Ram Configuration per NOP/WSP
Check for short bolting on BOP components prior to rig up
Flange connection only-BOP to Wellhead
INJECTOR HEAD
Certified rigging on IH (slings, D-rings, Shackles, etc)
Inspection and weight rating documentation for pad eyes
IH chain guards properly secured, secondary tether installed
Keepers on bolts/lock nuts installed
Fall restraint not deployed (SRL Sala Block)
Hydraulic lines restrained (BOP, IH)
