Lifeline inspection checklist

If you discover any damage during inspection, immediately remove the lifeline from service by marking “NO” in the “Can the lifeline remain in service?” section. Document the specific damage using the checklist options, add photos as evidence, and include detailed comments about the issue. Then notify your safety manager about the equipment needing replacement.

Daily inspections can be performed by trained users of the equipment, but annual comprehensive inspections must be conducted by a “competent person” as defined by OSHA—someone who can identify hazards and has authorization to take corrective action. Your organization should designate and train specific individuals for this responsibility.

Always document the manufacturer name, serial number, and manufacture date as these details are essential for tracking service life, identifying recalled products, and following specific inspection guidelines. Some manufacturers specify maximum service life regardless of condition, making the manufacture date particularly important.

Self-retracting lifelines require additional checks for proper retraction function, braking mechanism operation, and impact indicators. Fixed lifelines need more focus on anchor points, tension, and cable integrity along the entire length. Both types require checking for physical damage, but the mechanical components of self-retracting models need special attention.
