Risk management breaks down without ownership and visibility
You can’t see what’s still open, so risks linger
Actions get “agreed” but are never finished
Everyone documents differently
How Lumiform works for dynamic risk assessments
Build structured assessments, complete them on mobile, trigger actions when hazards appear, and track every open risk to closure from one dashboard.
1. Build your risk assessment form

2. Complete the assessment on site

3. Assign hazards to an owner and track closure

4. Monitor open risks across sites in real time

Risk assessments completed, tracked, and closed
Multi-site teams need clear closure and accountability, because “sent an email” isn’t the same as “risk removed.”

Industries that run dynamic risk assessments in the field
These industries work across sites and shifts, so structured assessments and owned actions prevent hazards from staying open and resurfacing.

Construction
Supervisors run dynamic checks before and during work, capture evidence, and assign fixes with sign-off so changing site conditions don’t create unmanaged risk.

Manufacturing
Safety leads assess shift risks, flag hazards in the workflow, and assign actions before handover so the next shift inherits controls, not open issues.

Logistics & warehousing
Teams assess docks and storage areas, then track actions by site so managers see what’s overdue and what’s closed without digging through reports.

Facilities management
Technicians assess risk during callouts, work offline in low-signal buildings, then sync evidence so ownership and proof follow the job, not the person.

Oil, gas & energy
Field teams assess hazards in remote areas, capture proof offline, and sync later so risk decisions stay traceable even when there’s no connection.

Retail & food service
Managers run consistent checks across high-turnover locations, then approve closures with evidence so standards hold even when teams change.

Common questions
Lumiform covers the full range of occupational safety documentation, including Gefährdungsbeurteilungen. You build the risk assessment as a digital form using the no-code template builder, assign it to specific sites and responsible persons, schedule it to recur, and track completion across all locations from one dashboard. Conditional logic lets you branch into sub-questions based on responses. The platform handles inspections, audits, risk assessments, operating instructions, and permit-style workflows within the same system.
Yes. You configure conditional logic directly in the form builder so that a specific answer triggers follow-up questions, a sub-section, or an automatic corrective action. If a field worker ticks “No” on a PPE item, the form can immediately surface additional questions, require a photo, or create an action assigned to the responsible person. No manual routing needed. The logic is set once in the template and applies consistently across every inspection completed by every team.
Yes. The Lumiform mobile app works fully offline on iOS and Android. Field workers start an inspection with a connection, go underground or into any area without signal, and continue completing the form without interruption. All data saves locally on the device. Once the device reconnects to a network, the inspection syncs automatically to the platform. Operations managers see the results in the dashboard as soon as the sync completes. No data is lost during the offline period.
Yes. The analytics dashboard shows overdue forms by site, group, or responsible person without clicking into individual records. You can filter to see exactly which construction sites have not completed their risk assessment, who is responsible, and how long they are overdue. Lumiform sends automatic email notifications to non-compliant sites based on the schedule you configure. You do not need to chase people manually or export a spreadsheet to find out who is red.
Yes. When a corrective action passes its due date without being closed, Lumiform automatically sends a notification to the responsible person. You configure the escalation and notification rules when setting up the workflow. This removes the need for manual follow-up emails or verbal reminders. The action remains visible in the dashboard as overdue, so operations managers can see outstanding items across all sites without having to contact each person individually to check status.
Yes. When you assign a corrective action to an employee, they receive a notification and can open the action on their mobile device. They complete the required steps, attach photos or files as evidence, and mark it done. The action history records every update, including who made changes and when. If you have approval workflows active, the action stays open until the designated approver confirms the evidence and closes it. The full audit trail is stored and accessible at any time.
This depends on how you configure the workflow. With the Approvals add-on active, you set up a review step so that an action marked done by a supervisor remains in a pending state until the designated approver, such as an account manager or safety officer, reviews the evidence and confirms closure. Without an approval step configured, the action closes when the responsible person marks it complete. The approval module is available as an add-on on the Professional plan and is included in the Enterprise plan.
Yes. Lumiform supports attaching documents to forms and actions, not only photos. Field workers and managers can attach files such as PDFs, manuals, or specification sheets directly within the platform. This means a corrective action can include supporting documentation alongside photos, so the person responsible has everything needed to address the issue without switching to email or another system. You can also attach documents to individual form questions, which is useful for safety manuals or regulatory guidelines.
The approval module is available as an add-on on the Professional plan or included in the Enterprise plan. Whether it is active from day one depends on which plan you start with during the pilot. If you want to test approval workflows during your two-month build and configuration phase, confirm with the Lumiform sales team that the add-on is included in your trial or pilot agreement. The module covers both form approvals and action approvals, so you can test the full workflow before go-live.
You build permit-to-work style forms using the template builder, adding the required fields, signatures, and conditional logic for confined space or electrical work scenarios. The approval workflow routes the completed permit to the designated approver before work begins. E-signatures are supported natively. Offline capability means workers can complete the permit in areas without signal. This keeps the permit process inside one platform rather than running it through email or paper, and creates a traceable record of every permit issued and approved.
