GENERAL CHECKLIST

Has the area around the scaffold been secured and safety signs placed?

Are all the sufﬁcient scaffold elements in place and without any defects?

Are all scaffold legs supported and braces correctly attached to the scaffold?

Is a qualiﬁed, responsible person in charge of the erection/inspection?

Is the scaffold adjusted to the structure to prevent movement?

Are the base plates in ﬁrm contact with mudsills and the frame of the scaffold?

Is a toe board accurately placed when necessary?

Are all scaffold components and planking in an adequate condition?

Are brackets, tubes, planks & accessories correctly installed with wedges tightened?

Is the whole scaffold even and plumb?

Are mud sills correctly placed and appropriately sized where ever needed?

Were screw jacks used to even and plumb the scaffold whenever demanded?

Has there been a check to verify clamps are secured in place?

Are all planks properly graded?

Is there appropriate access to get on and off the scaffold?

Is the ladder secured and in place?

Has "Don't Use" tag been set at all access points where inspection failed?

Has the scaffold control tag been signed and certified for this particular use?

Are all necessary guard rails available on all open sides and ends?

Do all planks have a minimum of 12" overlap and extend 6" exceeding supports?
