Annual inspections of full body harnesses are essential for workplace safety and regulatory compliance. This template takes you through a thorough examination of harness identification details, hardware condition, webbing integrity, and more.

According to OSHA regulations, personal fall arrest systems must go through annual inspections as well as regular pre-use inspections. The template helps you document these inspections systematically, providing clear criteria for determining whether equipment should remain in service or be removed. HARDWARE
D-rings Check D-rings for rust or corrosion, distortion, cracks, breaks, and rough or sharp edges. D-rings should pivot freely.
Buckles note any unusual wear, damage or distortion. On tongue buckles check that the roller and tongue move freely and that the tongue overlaps the buckle frame. Check the outer and center bars on friction and mating buckles for distortion. On quick release types check for broken plastic and that they will not release when one release is opened. Mating buckles should nest together.
WEBBING
Webbing Inspect the entire length of all the webbing looking for frayed edges, broken fibers, pulled stitches, cuts, burns and chemical damage. Also look for mildew, discoloration, hard shiny spots, any webbing that has been marked with a felt pen, excessive hardness or brittleness or the Harness Fall Indicator deployment.
Stitching Check all stitching for ripped or pulled stitches and to make sure the webbing joints are not loose. Also look for missing stitching, hard or shiny spots, or discoloration of stitching.
LABELS
Inspect to be certain that all harnesses have a label that is legible and securely held in place.
OVERALL DISPOSITION
Harness is approved for use at time of inspection.
Harness is NOT approved for further use and must be removed from service.

When should I remove a harness from service based on inspection results?

Remove a harness from service immediately if you find cuts, tears, abrasion, mold, excessive soiling, chemical damage, or burn marks on the webbing. Also remove it if hardware shows cracks, deformation, or corrosion; if stitching is loose, broken, or missing; if labels are illegible or missing; or if the harness has been involved in a fall. When in doubt about any component’s integrity, err on the side of caution and remove the harness.

How do I properly document a failed inspection in this template?

Mark the specific component that failed as “AT RISK” in its corresponding section, then mark “YES” in the “Harness is NOT approved for further use” field. Add detailed notes describing the exact nature of the failure, including measurements and location of damage if possible. Take photos of the damaged areas for your records. Complete all identification fields to ensure the specific harness is properly documented and can be tracked through the removal process.

What training does the person conducting annual harness inspections need?

The inspector must qualify as a “competent person” under OSHA regulations, meaning they can identify hazards and have authorization to take corrective action.
